Job summary

JourneyCare Hospice in Chicago is seeking a compassionate Hospice RN Case Manager to provide patient-centered, end-of-life care in the Chicago area. Schedule Monday–Friday, 8:30 AM – 5:00 PM with one weekend rotation every 8 weeks; territory includes Hyde Park, Evergreen Park and surrounding communities.

The role offers a salary of $80,000–$97,000 per year, plus benefits such as 401(k) matching, medical/dental/vision, PTO, and hospice certification support with potential salary increases.

Qualifications

• Graduate of an accredited RN program.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver’s license and proof of insurance.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Supervise Home Health Aides in compliance with regulations.
