Job summary

Heartland Hospice is seeking an experienced Registered Nurse for full-time hospice case management. You will perform in-home assessments, deliver skilled care, and coordinate with an interdisciplinary team to tailor plans of care for patients with life-limiting illness.

You will educate families on pain management, symptom control, and end-of-life processes, supporting emotional and spiritual needs while ensuring regulatory compliance and quality documentation.

Qualifications

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Strong communication and time management skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care with IDG and physicians to ensure seamless services.
• Educate patients and caregivers on disease processes and end-of-life care.
