Job summary

Generations Hospice is seeking a compassionate RN Case Manager to deliver patient-centered hospice care in Fond du Lac, WI. You will work within a strong interdisciplinary team to assess needs, develop care plans, and administer nursing interventions, while maintaining clear documentation and family education.

Qualifications

• Graduate of an accredited nursing program with current WI licensure.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
