Generations Hospice is hiring a Registered Nurse Case Manager to provide compassionate hospice care across Beaver Dam, Mayville, and Fond du Lac, WI. The role offers a $7,500 sign-on bonus and a Monday–Friday, 8am–5pm schedule in a supportive, interdisciplinary team environment.

You’ll lead 1:1 patient care, develop individualized care plans, and collaborate with social workers, chaplaincy, aides, and providers. A driver’s license and Homecare Homebase EMR experience are preferred.

Qualifikationen

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a vehicle in good working order.

Aufgaben

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
