St. Croix Hospice seeks a Registered Nurse Case Manager to deliver 1:1 holistic home-based care, partnering with an interdisciplinary team to develop care plans and guide end‑of‑life conversations with patients and families.

The role requires an RN with an accredited nursing program and 1+ years of acute care experience; hospice/home health experience is preferred. On-call duties and reliable transportation are part of the schedule.

Qualifications

• Qualified to practice as an RN (Associates/Bachelor’s/Master’s in nursing)
• 1+ years of recent acute care experience; hospice/home health experience preferred
• Ability to develop individualized care plans with patients and families
• Excellent communication and education skills for patients and caregivers

Responsabilités

• Provide 1:1 holistic home-based care to patients and families
• Develop and update individualized care plans with the patient and family
• Educate patients and caregivers on care techniques and medications
• Administer medications and manage symptom control as prescribed
• Rotate on-call duties and coordinate seamless care transitions
