Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to support patients and families in their homes.

You will lead 1:1 holistic care planning, collaborate with an interdisciplinary team, and guide end-of-life conversations with professionalism and empathy. Requirements include RN license with Associates, Bachelors, or Masters, and 1+ year acute care; hospice/home health experience preferred.

Qualifications

• RN license (any of the three degree levels) from an accredited program.
• 1+ years of recent acute care experience; hospice/home health preferred.
• Ability to perform comprehensive assessments and develop care plans.
• Educate patients and families on care techniques and medications.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family, updating as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
