Job summary

St. Croix Hospice in Moline, IL is seeking a dedicated Registered Nurse Case Manager to deliver 1:1 holistic care at patients' homes, partnering with an interdisciplinary team to create individualized plans and guide end-of-life conversations with empathy.

The role requires an RN from an accredited program, 1+ year acute care experience (hospice/home health preferred), on-call duties, and reliable transportation.

Qualifications

• RN license or current nursing license.
• 1+ years of acute care experience required; hospice/home health experience preferred.
• Develop individualized care plans with patient and family; update as needs evolve.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with the patient and family, updating as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
