Job summary

Community Healthcare of Texas is seeking an RN Case Manager to meet with patients and families, coordinate admissions, and manage ongoing plans of care in a hospice setting.

The role includes developing care plans, administering treatments, and coordinating with physicians and the hospice team. Texas RN license and valid driver’s license are required; 1 year nursing experience and hospice case management exposure preferred.

Qualifications

• Must be a Texas-licensed RN.
• Must have a valid driver’s license and reliable transportation.
• 1 year of nursing experience.
• Case management experience (hospice).
• 2+ years hospice case management strongly preferred.
• Proficient computer skills; MS Office experience preferred.

Responsibilities

• Meet with patients and families to explain hospice scope of services and coordinate admission.
• Develop and revise plan of care with patient/family involvement.
• Provide nursing care using the nursing process.
• Administer medications and treatments as prescribed.
• Act as Case Manager and coordinate care for assigned caseload.
• Document clinical notes and track pain/symptom management changes.
