Job summary

Trinity Health's Saint Agnes Home Health and Hospice in California is seeking a Hospice Registered Nurse (RN) Case Manager to perform comprehensive assessments, develop personalized care plans, and deliver compassionate care at patients' homes in alignment with the interdisciplinary team. This full-time role offers guaranteed hours, a $10,000 sign-on bonus, Day One benefits, and a competitive pay range.

Applicants should have an active California RN license and at least 1 year of nursing

Qualifications

Current RN license (BSN preferred).
Minimum 1 year of nursing experience (required in California).
Home health care experience preferred.
Valid driver’s license and reliable transportation.
Strong computer skills and a commitment to ethical, mission-driven care.

Responsibilities

Provide skilled nursing care per physician orders and organizational policies.
Perform comprehensive assessments and develop individualized care plans.
Collaborate with physicians, therapists, social workers, and home health aides.
Supervise and support LPNs and Home Health Aides.
Ensure compliance with regulatory standards and contribute to quality improvement initiatives.
Promote patient satisfaction and positive health outcomes.
