Job summary

Trinity Health At Home is seeking a Hospice RN Case Manager to provide professional nursing care for hospice clients, including assessment, care planning, interventions, and supervision of care outcomes. This role operates within the hospice program under physician guidance and state nursing regulations.

The ideal candidate holds an RN license, with acute care or hospice experience, and demonstrates strong communication skills and compassionate end-of-life care.

Qualifications

• Graduate of an approved Nursing education program.
• Current Registered Nurse licensure in the state of practice.
• Must have a minimum of one (1) year experience as a professional acute care nurse.
• Home healthcare/hospice experience preferred.

Responsibilities

• Informs the client/caregiver about agency, care/services, payment responsibilities and rights before care is initiated.
• Develops and implements an individualized Plan of Care with outcome-focused goals.
• Makes referrals for evaluation/care to other disciplines and coordinates care with others.
