Job summary

Constellation Health Services in Connecticut seeks a compassionate Registered Nurse Case Manager to coordinate hospice and home health care across Norwalk, Stamford, and Greenwich. You will assess patient needs, manage medications and treatments, and collaborate with families on the care plan.

The role requires RN licensure in CT, 1–2 years of RN experience, and hospice experience is preferred. Basic iOS knowledge for documentation is expected; weekend/holiday coverage may be needed.

Qualifications

• Graduate of an accredited RN program.
• Registered Nurse Licensure in the state of employment.
• At least 1-2 years of RN experience is a must.
• Hospice care experience strongly desired.
• Basic knowledge of iOS operating system.

Responsibilities

• Provide skilled nursing care and administer medications per the plan of care in patients’ homes.
• Perform comprehensive assessments and coordinate the plan with family and care team.
• Observe and report changes to the nurse supervisor and physician.
• Participate in weekly interdisciplinary team meetings with updates.
• Maintain documentation according to agency standards and timelines.
• Use a computer-based system to submit required documentation and time records.
• Support hospice aides’ evaluation as needed.
• Occasional weekend/holiday coverage as required.
