Job summary

Emblem Hospice in Arizona is seeking a dedicated Hospice RN to serve patients in their homes and communities. As a trusted Case Manager you will guide families through care decisions, delivering comfort, dignity and compassionate support during life’s tender moments.

This full-time role covers Scottsdale and Paradise Valley and offers an annual salary of $85,000–$95,000+, dependent on experience, plus ongoing education, mentorship, and a collaborative team that values your growth.

Qualifications

• Arizona RN license required
• Minimum 2 years nursing experience, hospice or home health preferred
• HCHB experience preferred but not required

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs
• Develop and implement individualized care plans, revising as needed
• Provide skilled nursing care, administer medications, and manage symptoms
• Educate patients and families on care techniques, medications, and comfort measures
• Maintain accurate clinical documentation and communicate changes to physicians
• Participate in interdisciplinary team meetings and quality improvement initiatives
• Provide emotional support and counseling to patients and families
• Supervise hospice aides and delegate responsibilities appropriately

Role

Requirements

• Currently licensed as a Registered Nurse (RN) in the State of Arizona
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) experience preferred, not required
• Valid driver’s license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
