Job summary

Emblem Hospice in Phoenix seeks a seasoned social work leader to supervise and mentor a team of Licensed Social Workers, oversee onboarding and professional development, and drive high standards of patient-centered care.

You will ensure psychosocial assessments and care planning, collaborate with interdisciplinary colleagues, support families facing end-of-life challenges, and uphold Medicare/Arizona compliance through quality initiatives and documentation.

Qualifications

  • MSW from CSWE-accredited program.
  • Active Arizona LMSW or LCSW license.
  • Min 3 years professional social work in hospice/palliative care.
  • Min 1 year leadership/management experience.
  • Valid Arizona driver's license and reliable transportation.

Responsibilities

  • Supervise, mentor, and support a team of Licensed Social Workers.
  • Assist with onboarding, training, coaching, and professional development.
  • Conduct performance reviews and provide ongoing guidance.
  • Serve as a clinical resource for complex patient and family situations.
  • Promote best practices and high standards of care across the social work team.
  • Oversee psychosocial assessments and care planning activities.
  • Collaborate with nurses, physicians, chaplains, aides, and other team members.
  • Support patients and families facing end-of-life challenges.
  • Assist with crisis intervention, counseling, and resource coordination.
  • Ensure services are delivered with empathy, respect, and cultural sensitivity.
  • Help ensure compliance with Medicare Conditions of Participation and regulations.
  • Monitor documentation quality and regulatory compliance.
  • Participate in quality improvement initiatives and audits.

  • Ensure services are delivered with empathy, respect, and cultural sensitivity.
Quality & Compliance
  • Help ensure compliance with Medicare Conditions of Participation, Arizona regulations, HIPAA requirements, and hospice industry standards.
  • Monitor documentation quality and regulatory compliance.
  • Participate in quality improvement initiatives, audits, and survey readiness activities.
  • Support operational excellence while maintaining exceptional patient-centered care.
Required Qualifications
  • Master's Degree in Social Work (MSW) from a CSWE-accredited program.
  • Current, active Arizona social work license in good standing: LMSW or LCSW accepted.
  • Minimum of three (3) years of professional social work experience in hospice, palliative care, healthcare, home health, or a related clinical setting.
  • Minimum of one (1) year of leadership, supervisory, or management experience.
  • Strong understanding of psychosocial assessment, family dynamics, and end-of-life care.
  • Valid Arizona driver's license, reliable transportation, and current automobile insurance.
Preferred Qualifications
  • Hospice experience strongly preferred.
  • Previous experience supervising social workers.
  • Knowledge of Medicare hospice regulations and interdisciplinary team practices.
  • Bilingual candidates are encouraged to apply.
