Emblem Healthcare in Chandler, AZ is seeking an experienced Home Health RN to bring clinical expertise and compassion into patients' homes. You will build meaningful relationships, create personalized care plans, and coordinate care as a trusted Case Manager in the East Valley (Mesa, Chandler, Gilbert, Queen Creek).

Qualifications

• Must hold an active Arizona RN license.
• Minimum of 2 years nursing experience, preferably in Home Health, Emergency, ICU, or MedSurg.
• Strong clinical skills and a patient-centered approach.

• This is a full-time position providing service in the East Valley (including Mesa, Chandler, Gilbert, Queen Creek)
• Annual salary range $85000 - $90000 per year (depending on experience), plus travel expenses

As a Home Health RN, you’ll be the heart of patient care, bringing clinical expertise and compassion directly into the home.

Requirements:

• Currently licensed as a Registered Nurse (RN) in the State of Arizona
• Minimum of 2 years nursing experience, preferably in Home Health, Emergency, ICU, or MedSurg
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and make a real difference
• Valid driver’s license and reliable, insured vehicle
