Job summary

Trinity Health Mid-Atlantic is seeking a part-time Hospice Case Manager in Monticello, IA. The RN will provide professional nursing care for hospice clients, assess needs, plan care, and supervise outcomes in accordance with licensure and organizational policies.

Responsibilities include coordinating with the care team, explaining benefits, and developing a patient-centered plan of care. A sign-on bonus of $5,000 is offered for part-time positions; PRN does not include the bonus.

Qualifications

• Graduate of an approved Nursing education program.
• Current Registered Nurse licensure in the state of Iowa.
• Must have one year of professional nursing experience by regulation.
• Must have a minimum of one (1) year experience as a professional care nurse.
• Home hospice experience preferred.

Responsibilities

• Maintains current knowledge of applicable laws, the Integrity Program, Code of Ethics, and agency policies.
• Informs client/caregiver about the agency, services, payment obligations, and rights before care is initiated.
• Accurately explains/understands the Hospice Benefit.
• Develops and implements an individualized Plan of Care with attainable, outcome-focused goals.
• Makes referrals for evaluation/care and coordinates care with other disciplines to ensure effective service.
• Uses interview, observation and evaluation to assess clients and apply nursing judgment in planning interventions.
