Above & Beyond Home Health Care and Hospice Agency in Monticello, IA, a Trinity Health Company, seeks a Hospice Case Manager RN to deliver professional nursing care to hospice clients under physician guidance and agency policies. The RN will serve as case manager, assess needs, plan care, and supervise overall patient outcomes in the Monticello area.

Requires Iowa RN licensure and at least one year of professional nursing experience; home hospice experience preferred.

Qualifikationen

• Graduate of an approved Nursing education program.
• Current Registered Nurse licensure in the state of Iowa.
• One year of professional nursing experience by regulation.
• Minimum of one year experience as a professional care nurse.
• Home hospice experience preferred.
• Must have current Driver’s license and reliable transportation to and from work site.

• Maintains knowledge of applicable laws and policies and adheres to ethical and professional standards.
• Informs the client/caregiver about services, payment obligations, and rights before care is initiated.
• Develops and implements an individualized Plan of Care with outcome-focused goals.
• Makes referrals for evaluation/care to other disciplines and coordinates care with others.
• Assesses clients and uses nursing judgment to guide interventions and recommendations.
