Above & Beyond Home Health Care and Hospice Agency, located in Monticello, IA, is seeking a Hospice Case Manager RN to provide professional nursing care for hospice clients as prescribed by the physician and within licensure requirements. Qualifications

• Graduate of an approved Nursing education program.
• Current Registered Nurse licensure in the state of Iowa.
• Minimum of one year professional nursing experience.

Responsibilities

• Provide professional nursing care to hospice clients as prescribed by the physician and in compliance with licensure requirements.
• Develop and implement individualized care plans to improve quality of life and dignity.
• Coordinate care with other disciplines and services to ensure effective care delivery.
• Perform nursing assessments, care planning, and supervision of client care and outcomes.
• Guide families about hospice benefits and agency services before care begins.
