Job summary

Concho Hearts Hospice is seeking a PRN RN to provide compassionate hospice care in the Beaumont/Orange area, visiting patients in their homes and building real relationships with families.

You will manage a personal caseload, complete visits, admissions, recerts, and real-time charting on a tablet, working with an interdisciplinary team. Texas RN license required; hospice/home health experience preferred, training provided.

Qualifications

• Texas RN license.
• Hospice or home health experience preferred (we will train the right nurse).
• Strong communication and critical thinking skills.
• Reliable transportation for local travel.
• A nurse with heart, compassion, and independence.

Responsibilities

• Manage a caseload of hospice patients in the Orange area.
• Provide compassionate, skilled nursing care in patients’ homes.
• Complete visits, admissions, recerts, and documentation.
• Collaborate with a supportive interdisciplinary team.
• Help patients and families navigate end-of-life care with dignity.
• Chart in real time using a provided tablet.
