St. Mary’s Home Care and Hospice in Athens, GA seeks a part-time PRN Hospice RN Case Manager to provide professional nursing care to hospice clients in home settings.

The RN serves as case manager, assessing needs, planning care, and coordinating with physicians and the interdisciplinary team. Qualifications

• Graduate of an approved nursing education program.
• RN licensure in the State of Georgia.
• 1 year of professional acute care nursing experience.
• Home care experience preferred but not required.

Responsibilities

• Provide one-to-one nursing care to hospice patients in their homes.
• Collaborate with interdisciplinary team and attend weekly meetings.
• Coordinate with physicians and hospice director to manage symptoms and care plans.
• Support Aides and LPN/LVN staff as needed.
