Job summary

Sanctuary Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our nursing team delivering end-of-life care across Summit, Portage, Medina, Stark and Wayne counties. You will provide hands-on nursing care, coordinate services, and guide patients and families through their hospice journey.

The role requires an active RN license, at least 1 year of medical-surgical/acute care experience (hospice a plus), EMR proficiency, CPR as required, and a valid driver’s license with reliable

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical-surgical/acute care setting; hospice experience a plus.
• Must be computer proficient in typing and various programs, including EMR.
• Possess and maintain CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Collaborative Care Champion: Partner with our Clinical Director to assess eligibility and suitability of clients for our hospice services.
• Nurturing Direct Care: As an RN Case Manager, deliver hands-on nursing care and coordinate services to ensure comfort and quality of life.
• Nursing with Heart: Apply nursing skills to ease pain and bring solace rooted in proven principles.
• Educator and Advocate: Educate patients and families to create personalized care plans aligned with goals.
• Call of Compassion: Share call duties to be there when it matters most.
