Job summary

Athena Home Health & Hospice is seeking a Hospice RN Case Manager to plan, organize, and direct hospice care in patients' homes and communities. You will supervise aides, perform comprehensive assessments, and develop care plans aligned with nursing diagnoses and palliative actions.

You will also counsel patients and families, ensuring understanding of nursing needs, and serve as Case Manager for assigned caseload under the Clinical Supervisor.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years acute care experience; 1–2 years home care experience.
• Active RN license in Connecticut and CPR certification.
• Bachelor's degree; hospice care experience preferred.

Responsibilities

• Supervise hospice aides and provide professional nursing care using the nursing process.
• Complete comprehensive patient/family assessments and regular re-evaluations.
• Develop and implement care plans with goals and palliative actions.
• Counsel patients and families and provide health care instruction as appropriate.
• Act as Case Manager and coordinate care for assigned caseload.
