Crescent Hospice is seeking a Per Diem Hospice Registered Nurse Case Manager in Greenville. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through the hospice journey.

The role requires RN licensure, EMR proficiency, CPR certification, and a valid driver's license. Schedule is Monday–Friday, 8 am–4:30 pm, per diem; mileage and benefits are included where applicable.

Qualificações

• Must hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.

Responsabilidades

• Empower through engagement with compassionate care.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Nurture direct care: coordinate care while ensuring comfort and quality of life.
• Practice nursing with heart to ease pain and bring solace.
• Educate and advocate for patients and families to participate in care plans.
• Share call duties as part of the team.
