Hospice of North Carolina is seeking a Full Time Registered Nurse Case Manager (RNCM) to join our compassionate nursing team. You will deliver hands-on hospice care, coordinate services, and guide patients and families through the hospice journey.

The role requires an RN license and at least 1 year of RN experience, with EMR proficiency and willingness to participate in on-call duties. Territory covers Cherokee, Graham and Swain counties in North Carolina.

Qualificações

  • Hold current unencumbered RN license.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting; hospice experience a plus.
  • Must be computer proficient with typing and EMR systems; CPR certification if required by state.
  • Current driver’s license, vehicle insurance, and reliable transportation.

  • Provide hands-on hospice nursing care and coordinate services.
  • Collaborate with Clinical Director to assess eligibility and suitability of clients for hospice services.
  • Educate and advocate for patients and families to create personalized care plans.
  • Deliver nursing care rooted in proven principles to ease pain and enhance quality of life.
  • Respond to clinical referrals with compassion and professionalism.
  • Share on-call responsibilities as part of the team.

Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Hospice of North Carolinais seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 4:30 pm. Occassional On Call

Territory: Cherokee, Graham and Swain County

In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Responsibilities
  • Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
  • Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.
  • Nurturing Direct Care: As an RN Case Manager, you're not just coordinating care – you're a source of comfort and support for patients, ensuring their comfort and quality of life remain paramount.

Qualifications
  • Hold current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.
  • Current driver's license, vehicle insurance, and access to dependable transportation.

$85,000 - $87,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
