Job summary

Del Corazon Hospice is seeking a PRN Hospice RN to provide compassionate, patient-centered end-of-life care in Albuquerque. You will work with an interdisciplinary team to develop and implement care plans, manage pain and symptoms, and support patients and families.

This per diem role offers flexible scheduling and an opportunity to contribute to high-quality hospice care. Requirements include NM RN licensure, BCLS, and the ability to meet the on-call and visitation demands.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in New Mexico.
• One of: acute care, home health, community health/public health, or hospice experience in last two years.
• BCLS certification.
• TB testing current or can obtain day one; background check.
• Valid NM driver''s license and reliable vehicle; proof of auto insurance.

Responsibilities

• After-hours on-call as primary responsibility.
• Triage calls and visits as clinically warranted.
• Pronounce expired patients.
• Collaborate with IDT after on-call to report findings.
• Flat on-call rate plus per-visit rate.
• Collaborate with patients, families and the care team to coordinate care.
• Educate patients and families on diseases, treatments, and self-care.
• Document care and communicate with physicians and team members.
