Job summary
Trustbridge Hospice, a member of Empath Health, seeks an RN Hospice Case Manager to coordinate care for patients and families in the Boca area. You will work under the primary physician and with the interdisciplinary team to develop and implement personalized plans of care, monitor conditions, and deliver education to improve comfort and quality of life.
The role requires FL RN licensure, BLS, and at least 1 year in hospice or related nursing fields; EMR experience is a plus, with strong
Qualifications
- RN licensure in Florida required.
- Minimum 1 year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
- Understanding of Hospice Nursing: case management, crisis intervention, communication, teaching, thanatology.
Responsibilities
- Serve as an advocate for patients and families, ensuring needs are recognized and addressed.
- Coordinate care under the direction of the primary physician and with the interdisciplinary team.
- Develop and implement individualized plans of care as part of the IDG.
- Monitor patient conditions, provide interventions, and educate to enhance comfort and quality of life.
