Job summary

Trustbridge Hospice, part of Empath Health, seeks an RN Hospice Case Manager in Boca Raton for PRN hours. You will coordinate patient and family care under the primary physician and IDG, identify needs, and support comfort and quality of life at home or in facilities.

Ideal candidates hold an active Florida RN license with BSN preferred, hospice certification preferred, and BLS. Strong communication and organization skills are essential for success in this role.

Qualifications

• Florida RN license required with active status.
• BSN preferred; hospice certification is a plus.
• BLS certification required; experience in hospice is preferred.

Responsibilities

• Coordinate patient/family care under physician and IDG direction.
• Identify patient/family needs and develop care plans.
• Provide teaching and support to maximize comfort and quality of life.
• Maintain confidentiality and promptly address concerns.
