Trustbridge Hospice, a member of Empath Health, seeks a compassionate Hospice Registered Nurse (RN) Case Manager for Palm Beach County. The role runs Monday–Friday 8:30am–5:00pm with weekend requirements, guiding families through life’s journey with a patient-centered approach.

You will coordinate care with the IDG, develop individualized plans, monitor conditions, educate families, and address concerns to sustain high trust and comfort in patients’ final stages.

Formación

• Current Florida RN license and BLS certification required.
• 1 year in public health, home health, oncology, med/surg, geriatric, or hospice nursing preferred.
• Strong case management, crisis intervention, communication, teaching, and thanatology skills.

Responsabilidades

• Advocate for patients and families, ensuring needs are addressed.
• Coordinate care under the primary physician and with the IDG.
• Develop and implement individualized care plans as part of the IDG.
• Monitor patient conditions, provide interventions, and educate to improve comfort and quality of life.
• Address concerns promptly to maintain high satisfaction and trust.
