Job summary

Beacon Hospice, part of Amedisys, is seeking a dedicated RN Case Manager to coordinate patient care in the Rhode Island service area including Newport and South County. You will assess patients, develop plans of care, and collaborate with the care team to ensure comfort and quality at home.

Ideal candidates have at least one year of nursing experience, an active RN license for the state, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• One year of nursing experience.
• RN license (state-specific).
• CPR certification is current and valid.
• Valid driver's license, reliable transportation and insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
