Job summary

Beacon Hospice, Amedisys in Newport, RI seeks an experienced RN case manager to coordinate care for patients at home. You will perform patient assessments, develop and implement plans of care, and collaborate with the care team to ensure comfort and quality outcomes.

In this role you’ll provide on-call coverage as needed and educate patients and families on care techniques, medications, and symptom management. Join a trusted leader in home health and hospice services.

Qualifications

• One year of nursing experience.
• Current RN license (state-specific).
• CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to plan care.
• Makes referrals to other hospice disciplines as needed or documents rationale.
• Maintains individualized patient care progress records accurately.
• Records pain/symptom management changes/outcomes and reports them.
• Teaches patients, families, and caregivers proper patient care techniques.
• Communicates regularly with clinical manager and care team.
• Performs on-call duties for patients and families as assigned.
• Other duties as assigned.

Attractive pay

• $92,000.00 - $100,000.00 salary

• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications

• One year of nursing experience.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
