Job summary

Phoenix Home Care & Hospice is seeking a Registered Nurse (RN) Home Health Case Manager in Duenweg, Missouri. You will work with the interdisciplinary team to develop patient/family centered care plans and guide healthcare decisions to improve quality of life.

The ideal candidate will bring leadership, strong organizational and interpersonal skills, and the ability to provide compassionate, precise guidance in demanding situations. Missouri RN licensure and valid driver’s license are required.

Qualifications

• Missouri or Compact RN License.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Provide education to patients/family regarding the process, plan of care, and guidance on healthcare decisions.
• Consult with the attending physician and Hospice Medical Director regarding changes in patient condition requiring changes to the plan of care.
• Coordinate hospice care with team members and with patients/family to ensure needs are met.
• Conduct initial and ongoing comprehensive assessments.
• Perform evaluations for hospice care and informational visits.
