Qualifications

• Current, active RN license (compact/multistate preferred).
• Minimum 2 years of nursing experience, with hospice, palliative care, home health, geriatrics, oncology, or related field.
• Experience providing direct care to patients with life‑limiting illnesses.
• Strong assessment, critical‑thinking, and clinical decision‑making skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently with minimal supervision while collaborating within an IDG.
• Computer proficiency and comfort with electronic documentation systems.
• Valid driver's license and reliable transportation for home visits.

Responsibilities

• Serve as the primary nurse responsible for coordinating all aspects of patient and caregiver care.
• Conduct comprehensive nursing assessments to identify physical, emotional, psychosocial, and spiritual needs.
• Develop, implement, evaluate, and update individualized plans of care in collaboration with the interdisciplinary group (IDG).
• Provide direct nursing care, symptom management, and clinical interventions in accordance with hospice standards and protocols.
• Promote patient and caregiver independence whenever possible.
• Provide compassionate education and support to patients and families, ensuring understanding of care plans, medications, and disease progression.
• Maintain ongoing communication with physicians, team members, and caregivers to ensure continuity of care.
• Respond promptly to changes in patient condition and adjust care plans accordingly.
• Participate in on‑call rotation as assigned to ensure 24/7 patient support.
• Attend and actively participate in IDG meetings and case conferences.
• Collaborate with social workers, chaplains, CNAs, volunteers, and other team members to address patient and family needs.
• Identify when additional services or disciplines are needed and initiate referrals appropriately.
• Maintain strong, respectful working relationships with all team members.
• Complete accurate, timely, and concise clinical documentation in accordance with organizational policies.
• Ensure all care delivered meets state, federal, and accreditation standards.
