Job summary

Lotus Blossom Hospice and Palliative Care in Georgia seeks a dedicated Registered Nurse Case Manager to coordinate patient care within our hospice team. The role focuses on comfort outcomes, family and provider communication, and symptom management to support patients at end of life.

The ideal candidate holds an active GA RN license with hospice experience, strong documentation skills, and the ability to work collaboratively across disciplines.

Qualifications

• Active RN license in Georgia.
• Hospice experience required.
• Proficient in medical record management and documentation.
• Strong diagnostic evaluation and patient assessment.
• Ability to collaborate with a multidisciplinary team.
• Excellent communication skills with patients and families.
• Knowledge of infectious disease care protocols is a plus.
• Experience in palliative and chronic disease settings may be beneficial.

Responsibilities

• Coordinate patient care across the interdisciplinary team to ensure continuity of care.
• Conduct thorough diagnostic evaluations and assessments to develop individualized care plans.
• Manage medical records to ensure accuracy and confidentiality.
• Collaborate with multidisciplinary teams to provide comfort care for complex needs.
• Facilitate communication between patients, families, and providers about treatment plans.
• Monitor patient progress and adjust care plans based on outcomes.
• Educate patients and families regarding hospice management and available resources.
• Participate in quality improvement initiatives to enhance patient safety and care delivery.
• Will cover parts of Paulding, Bartow, south Cherokee Counties, Fulton; mileage reimbursement provided.
