Qualifications

• Current, active RN license and compact/multistate license preferred.
• Minimum 2 years nursing experience, with hospice or palliative care preferred.
• Strong assessment, clinical decision‑making, and communication skills.

Responsibilities

• Coordinate comprehensive end‑of‑life care for patients and families.
• Perform nursing assessments and develop individualized care plans.
• Provide direct nursing care, symptom management, and education.
• Maintain 24/7 patient support via on‑call rotation.
• Document accurately and ensure regulatory compliance.
• Collaborate with IDG members and referrals.
