Job summary

Superior Hospice is seeking a skilled Hospice RN Case Manager to deliver compassionate end-of-life care across the Greater Twin Cities Metro Area. You will manage assigned cases, coordinate care across settings, and support families with expert pain and symptom management.

You will collaborate with physicians and a multidisciplinary team, perform thorough nursing assessments, and ensure accurate EMR documentation while participating in weekend/on-call rotations.

Qualifications

• Active Minnesota Registered Nurse (RN) license.
• Experience in hospice preferred, or oncology/ICU/palliative care/home health or other complex clinical settings.
• Strong clinical judgment and ability to work independently.
• Exceptional communication and relationship-building skills.
• A passion for patient-centered, compassionate hospice care.
• Reliable transportation and willingness to travel throughout your assigned territory.

Responsibilities

• Manage an assigned hospice caseload throughout the continuum of care.
• Provide expert pain and symptom management focused on comfort and quality of life.
• Serve as the primary Hospice RN Case Manager for patients and families.
• Complete comprehensive nursing assessments and individualized care plans.
• Educate and support caregivers through every stage of the hospice journey.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners.
• Provide care in patients' homes, assisted living communities, and skilled nursing facilities.
• Maintain accurate EMR documentation and participate in a weekend and on-call rotation.
