Job summary

Athena Home Health & Hospice is seeking a full-time RN Hospice Case Manager to cover Litchfield County and western Hartford County. This on-the-road role offers weekly pay up to $49 per hour, depending on experience, with a Monday–Friday 8:00 a.m. to 4:30 p.m.

shift. As an RN Hospice Case Manager, you will plan, organize, and direct hospice care within patients' homes and communities, supervising aides and coordinating with families and clinicians.

Qualifications

• Graduate of an accredited school of nursing.
• One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care.
• Active and unencumbered RN license in Connecticut.
• CPR certification.
• Bachelor’s degree, with one year of hospice care experience preferred.

Responsibilities

• Supervises hospice aides.
• Performs initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Develops and implements care plans based on nursing diagnoses.
• Provides nursing care and administers medications as prescribed.
• Coordinates patient care as Case Manager with the Clinical Supervisor.
