Athena Home Health & Hospice is seeking a compassionate Hospice RN Case Manager to plan, organize, and direct hospice care services in patients' homes and communities in Southern New England.

The role covers Litchfield County and western Hartford County, with a salaried position and weekly pay up to $49 per hour, depending on experience. Monday–Friday 8a–4:30p; relocation-driven vacancy.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years recent acute care in an institutional setting and 1–2 years in home care preferred.
• Active Connecticut RN license; CPR certification.
• Bachelor's degree preferred with hospice care experience.

Responsabilités

• Supervise hospice aides and provide professional nursing care using the nursing process.
• Complete comprehensive assessments of patients and families to determine hospice needs and re-evaluate regularly.
• Develop and implement care plans with goals based on nursing diagnosis and palliative actions.
• Counsel patients and families on nursing needs and provide health care instructions.
• Act as Case Manager to coordinate patient care for assigned caseload.
