Athena Health Care Systems is seeking a full-time RN Hospice Case Manager to join the Home Health & Hospice team serving the Torrington area. The role covers Litchfield County and western Hartford County, with a salaried position and weekly pay up to $49 per hour depending on experience.

The RN will plan, organize, and direct hospice care, building from community resources to meet patient and family needs in homes and communities. Shift is Monday–Friday 8:00 a.m. to 4:30 p.m.

Formación

• Graduate of an accredited school of nursing.
• 1–2 years of acute care experience in an institutional setting.
• 1–2 years of home care experience.
• Active Connecticut RN license.
• CPR certification.
• Bachelor’s degree with hospice care experience preferred.

Responsabilidades

• Supervise hospice aides.
• Complete initial and ongoing assessments of patient and family.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family.
• Coordinate patient care as Case Manager.
• Administer medications and treatments as prescribed.
• Counsel patient and family.
• Provide health care instructions.
• Assist with activities of daily living.
• Coordinate caseload as assigned.
