Job summary

Athena Home Health & Hospice is hiring a full-time RN Hospice Case Manager to serve Torrington and surrounding counties. The role is on-the-road, covering Litchfield County and western Hartford County, with weekly pay up to $49 per hour and a Monday–Friday 8a–4:30p shift.

The nurse plans, organizes, and directs hospice care, drawing on community resources to meet patient and family needs in homes and communities. Requires CT RN license and CPR certification; bachelor’s preferred.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of acute care in an institutional setting and 1–2 years in home care.
• Active and unencumbered RN license in Connecticut.
• CPR certification.
• Bachelor’s degree, with hospice care experience preferred.

Responsibilities

• Supervise hospice aides.
• Complete initial and ongoing patient/family assessments.
• Provide professional nursing care per the nursing process.
• Write and implement plan of care; regularly reevaluate.
• Initiate and revise the plan of care as patient needs change.
• Coordinate patient care as Case Manager with supervisor.
