Job summary

Vantage Hospice in Bryan, TX is expanding its hospice team and seeks an experienced RN Case Manager to join our compassionate care team. You will assess patients, develop individualized care plans, and coordinate services to deliver end-of-life care with empathy.

Texas RN license required; hospice experience preferred. We offer full-time hours, competitive salary, and a supportive interdisciplinary team focused on families and patients at the end of life.

Qualifications

• Current licensure as an RN in Texas.
• Hospice nursing experience preferred.
• Experience caring for terminally ill patients and families.
• Background in Oncology, Home Health, or Long-Term Care is a plus.
• Strong physical and psychosocial assessment skills; independence in clinical assessment.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized hospice care plans.
• Coordinate and manage patient care services while ensuring HIPAA compliance and regulatory guidelines.
• Collaborate with the interdisciplinary team to support discharge planning and post-acute needs.
• Educate and support patients and families regarding hospice services and end-of-life care.
• Maintain accurate, timely documentation of patient progress and interventions per agency policies.
