Hospice of South Texas is seeking an RN Case Manager to join our team in the Victoria/Port Lavaca area. The role involves comprehensive assessments, care planning, and education for patients and families dealing with terminal illness.

The position requires an active RN license and typically 2+ years of clinical RN experience, with hospice/home health preferred. On-call duties and occasional weekends may be required as part of a dedicated team.

Qualifications

• RN licensure is required and state license must be active.
• Two years of RN experience in clinical care preferred, hospice/home health favored.
• Strong interpersonal skills and nursing competencies essential.

Responsibilities

• Conduct initial and ongoing assessments of patient needs (physical, psychosocial, functional, environmental).
• Develop and revise the plan of care based on patient acuity and goals.
• Educate patients and families on disease processes, end-of-life care, and self-care techniques.
