Crossroads Hospice & Palliative Care is seeking an RN Case Manager to provide compassionate nursing care and coordinate services in home and facility settings in Bucks County.

You will build trusting relationships with patients and families, serve as the central communication point for the interdisciplinary team, and help patients live their final days with dignity.

Schedule is Monday through Friday, with competitive pay and benefits.

Qualifications

• Active RN license in good standing.
• 6 months of skilled nursing experience.
• Valid driver's license and auto insurance.
• Willingness to provide care in homes and facilities.
