Job summary

St. Mary's Home care & Hospice in Bucks County, PA, is seeking a full-time Hospice RN Case Manager.

The RN provides professional nursing care to hospice clients as prescribed by the physician and coordinates with families, following state Nurse Practice Act and organization policies. The role includes client assessment, care planning, symptom management, and supervision of care, with collaboration in an interdisciplinary team, weekly meetings, and interaction with the PCP or medical director.

Qualifications

• RN licensure in Pennsylvania
• Graduate of an approved nursing program
• Minimum 2 years nursing experience in acute care, home care or ER
• Home care experience preferred
• EPIC experience preferred

Responsibilities

• Perform hospice nurse case management with assessment, planning and care coordination
• Collaborate with interdisciplinary team and attend weekly meetings
• Coordinate with primary care physician or hospice medical director to manage symptoms
• Provide supervision and support to Hospice Aides and LPN/LVNs
• Deliver patient-centered care in patients' homes
