Job summary

Rocky Mountain Care Hospice is seeking a compassionate Hospice Registered Nurse (RN) to provide skilled, patient-centered care in the Davis/Weber area from Woods Cross, UT. This role emphasizes one-on-one patient care, comfort, and dignity for patients and families in a meaningful setting.

You’ll complete visits, assess patients, educate families, and coordinate with an interdisciplinary team while maintaining documentation and complying with scope of practice.

Qualifications

• Must have a current, unencumbered RN license.
• Certification in CPR.
• Prior clinical experience.
• Excellent communication skills.
• Demonstrated ability to maintain a high level of professionalism during stressful times.
• Strong critical thinking skills.
• Ability to adapt to different clinical settings and environments.

Responsibilities

• Complete hospice nursing visits and comprehensive patient assessments.
• Provide skilled nursing and comfort-focused care.
• Monitor changes in condition and respond to patient needs.
• Educate and support patients, families, and caregivers.
• Coordinate care with our interdisciplinary hospice team.
• Maintain accurate and timely clinical documentation.
• Make independent clinical decisions within your scope of practice.
• Oversee frequency of visits for the episode and coordinate with physicians as needed.
