Job summary

Brighton/Suncrest Hospice is hiring a Registered Nurse Case Manager to provide compassionate hospice care wherever patients call home, including private residences and facilities. You will collaborate with nurses, social workers, chaplains, CNAs, and volunteers to craft holistic care plans.

The role emphasizes lower clinician caseloads, strong support, and meaningful patient time, with opportunities for professional growth within a CHAP-certified hospice.

Qualifications

• Current Registered Nurse (RN) licensure in state(s) of practice.
• 1 year of hospice experience.
• Ability to travel across the service area with reliable transportation.
• Works well within a healthcare team and communicates clearly with patients and families.
• Proficient in phone, email and written communication and documentation.

Responsibilities

• Provide compassionate, individualized hospice care in patients' homes or care facilities.
• Collaborate with an interdisciplinary team to develop and adjust care plans.
• Support patients and families emotionally, physically, and spiritually.
• Maintain accurate documentation and care coordination.
• Manage visits and caseload with efficient time management.
