Job summary

Rocky Mountain Care Group is seeking a registered nurse to direct hospice care in home settings across Utah. You will assess patients, develop and revise care plans, administer prescribed treatments and educate families to support comfort and independence.

The role requires state licensure and CPR certification, with 1–2 years in acute care and home care preferred. Field-based responsibilities include on-call duties and collaboration with a multidisciplinary team.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of acute care in an institutional setting.
• 1–2 years of home care experience.
• Current licensure in state and CPR certification.
• Licensed driver with insured vehicle.

Responsibilities

• Plan, organize and direct hospice care in home settings.
• Complete patient assessments and plan of care.
• Coordinate patient care with interdisciplinary team.
• Educate and counsel patient and family.
• Provide on-call coverage as required.
• Supervise hospice aides and ensure quality of care.
