Responsibilities

• Oversee patient care after admission and assignment to RN Case Manager.

• Update the Plan of Care as patient status changes and needs evolve.
• Communicate with patients, families, and other providers; ensure documentation accuracy.
• On-call rotation to provide 24-hour service as assigned.
