Job summary

Advocate Health is seeking a Hospice RN Case Manager in Wisconsin to coordinate end-of-life care across settings. You will educate families, manage plans of care, and lead the Interdisciplinary Group to implement patient-centered services.

The role requires 1-2 years nursing experience in home health or hospice, WI/IL RN license, BLS, and driving ability; strong communication and critical thinking are essential for success.

Qualifications

• Licensed RN with active license in WI/IL; BLS certification required.
• 1-2 years nursing experience in home health, hospice, or related fields.
• Ability to develop and update care plans and document care accordingly.

Responsibilities

• Identify and coordinate patient/family care for terminally ill patients in hospice settings, with contact at least every 15 days.
• Educate patients and families on disease processes, hospice philosophy, and signs of decline; update physicians as needed.
• Review caseload to ensure appropriate services and level of care; administer medications as prescribed.
• Lead the IDG to develop and implement patient care plans; coordinate services and resources.
• Enter and update documentation in electronic health records per standards and policies.
• Provide ongoing assessment of treatment responses and adjust care; communicate changes to the team.
• Supervise Home Health Aide and LPN visits; instruct paraprofessional staff.
• Facilitate discharge planning and coordinate community resources for continuity of care.
• WI only: may conduct OASIS and hospice assessments; ensure plan of care adherence and safety.
• Adhere to safety procedures; maintain patient positioning, lifting, and body mechanics as required.
