Responsibilities

• Complete initial and ongoing assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care with patients and families
• Provide skilled nursing care including pain and symptom management and medication administration
• Educate patients and caregivers on disease progression, symptom management, and end-of-life expectations
• Coordinate care across caseload and liaise with patients, families, physicians, and care team
• Participate in IDG meetings to ensure holistic care
