Job summary

Elara Caring is seeking a Registered Nurse Case Manager, Hospice in Fort Wayne, Indiana. This salaried role supports hospice patients and families, coordinating care across settings and collaborating with an interdisciplinary team.

The ideal candidate holds an RN license in Indiana, has at least 1 year of clinical experience, and is willing to travel up to half of the time, with the ability to lift and transfer patients as needed.

Qualifications

• Graduate of an accredited School of Nursing (Certificate, Diploma, Associate, or Baccalaureate).
• Current State License as a Registered Nurse (RN).
• 1 year of clinical care experience.
• Experience in hospice or home health is preferred.
• 50% travel required.

Responsibilities

• Assess hospice care patients and families to identify needs and document findings.
• Communicate significant health and environment changes to the care team.
• Counsel, instruct, and include patient and family in the care plan.
• Teach hospice philosophy and pain/symptom management to staff in other settings.
• Provide bereavement assessment and support at time of death.
