Job summary

High Plains Senior Care Hospice is seeking a full-time RN Case Manager to lead hospice care planning for patients in their homes and communities in Amarillo, TX. You will coordinate interdisciplinary care, mentor Home Health Aides, and ensure accurate clinical documentation in a timely manner.

Responsibilities include admission assessments, care plan development with physicians, discharge planning, and maintaining patient confidentiality.

Qualifications

• Current Texas State license as a Registered Nurse.
• 2+ years’ experience as a Registered Nurse.
• Reliable transportation and valid Texas Driver's License.

Responsibilities

• Performs comprehensive assessment for patient admission.
• Develops and updates the plan of care in consultation with the physician and clinicians.
• Participates in the patient’s discharge planning process.
• Provides skilled nursing care as needed per the plan of care.
• Cooperates with other agencies to provide continuity of care and implement the care plan.
• Maintains ongoing professional development through in-service education and workshops.
• Maintains complete, timely clinical records and medication profiles.
• Maintains confidentiality of patients’ private health information.
